What information do I need to have ready before I enroll in a Medicare plan?

After enrolling in Original Medicare Part A and Part B, but before signing up for a specific plan, you will need to gather the following information:

  • ZIP code
  • Medicare number
  • Medicare effective dates
  • Last name
  • Date of birth
  • Bank account or credit card information (If you need to pay for a premium)


If you do not yet have your Medicare ID card, call the Social Security Administration, 1-800-772-1213 or 1-800-325-0778 (TTY). A representative should be able to tell you the Medicare effective dates.
