In rare cases, you may need to submit a Medicare claim (bill).
To do so, contact your physician and/or health care provider to file the claim.
Health care providers enrolled in Medicare are required to file Medicare claims for services covered under your plan.

When Do I Need to Submit a Medicare Claim?
Claims must be filed within one full calendar year of services rendered. Claims not filed within this guideline may result in non-payment from Medicare.

Can I Submit a Claim (Bill) to Medicare?
As the patient, you may also file claims by submitting the Patient Request for Medicare Payment Form (CMS-1490S)
But please note that service from a non-accepted Medicare service provider may result in patient self-pay to the service provider.
Do I Need to Submit a Claim (Bill) to Medicare if I Am Enrolled in a Medicare Advantage Plan?
If you have a Medicare Advantage (MA) plan, claims are filed with this plan. Medicare provides a monthly payment to specific private insurance companies so they can administer and manage your benefits. Please contact your Medicare Advantage plan administrator for more information about your benefits and guidelines under your plan.
