How do I get a new Medicare card if my card is damaged, lost or stolen?
To replace your Medicare card through the Social Security Administration, use one of the following three methods:

Replace a Medicare Card Online

Go to www.ssa.gov and log in to your My Social Security account.

Replace a Medicare Card in Person

Visit your local Social Security office to replace a Medicare card in person.  

Replace a Medicare Card by Telephone

Call 1-800-772-1213 (TTY users, dial 1-800-325-0778), Monday through Friday, from 7AM to 7PM.

Your new red, white, and blue card will be mailed to the address that Social Security has on file for you within 30 days.
